ContactInformation Collected Through the Contact Form (Questionnaire)
The contact form (questionnaire) collects your name, age group, email address, phone number (optional), your preferred way to be contacted and the best time to call, how you heard about us and the name of anyone who referred you, what you are looking for, the type of concern, your symptoms, since when, any diagnosis or treatment proposed elsewhere, how many clinics you have consulted, the kinds of records you have, whether you have felt unwell during dental anaesthesia or treatment and what happened, your wishes about treatment, your medical history, current medicines, allergies, pregnancy and breastfeeding, smoking and alcohol habits, family history, your occupation and work contact, an emergency contact, and the days and times that suit you for a visit. Items not marked “Required” are optional. Because information about symptoms is sensitive personal information, we ask for your consent before sending.
This information is used to reply to you and to prepare for your visit. Only if you consent to research use, it may be used, after removing information that identifies you such as your name, for research on occlusion and general health (conference presentations, papers, etc.). It is stored in the submission record we manage and in our email. It is not provided to third parties. Site analytics receive only the number of submissions, never the contents.
Notification emails to the clinic and confirmation emails to you are sent through the email delivery service Resend (Resend, Inc.; sending region: Tokyo), to which we entrust email delivery. The service handles the contents only to deliver the emails.
While you are filling in the form, your answers are saved temporarily only in your own browser (they are not sent to us) and are cleared when you send the form.
